First Case with da Vinci?
Give us a call at 800.874.7239 to set up your account or if you need technical assistance with your case.
Step 1: Insert Most Recent Rx Form.
Download the Rx prescription form, fill it out with the required information, and submit it together with the package.
Step 2: Wrapping
Wrap models individually and securely with bubble wrap. The additional wrap should be added to fill the box to avoid movement and possible breakage during shipping. If you need boxes or other shipping supplies, please click request lab supplies below.
Step 3: Documentation
Place the Rx prescription form and other documentation for our lab at the top of the box.
Step 4: Generate Shipping Label
Step 4: Secure the box and place the da Vinci pre-printed UPS label on the box. Please print shipping labels if you need them. (If you need more labels, you can also call the lab, not UPS).
Step 5: Schedule Pickup
Call UPS for a pickup at 1-800-PICKUPS or schedule a pickup online.
1-5 unit anteriors and posteriors
Full Rx prescription with instructions.
Photos required:
- Full face un-retracted
- Retracted lips
- Close up with shade tab
- Stump shade for all ceramic cases
6-10 unit anterior cases
Full prescription with instructions
Photos required:
- Full face un-retracted
- Retracted lips
- Close-up with shade tab
One of the following:
- Diagnostic wax-up
- Pre-op model and photo showing patient with pre-op smile
- Provisional model and photos of patient with provisional
Alternatively:
- The length of centrals and the smile guide style
One of the following are required:
- Stickbite
- Kois facial analyzer
- Facebow
Full Arch
Full prescription with instructions
- Close-up with shade tab
- Retracted lips
Photos required:
- Full face un-Retracted
One of the following:
- Diagnostic wax-up
- Pre-op model and photo showing patient with pre-op smile
- Provisional model and photos of patient with provisional
One of the following are required:
- Stickbite
- Kois facial analyzer
- Facebow
- Transfer Bite
